Physician owned hospice agency looking for compassionate and dedicated RN Hospice Nurse who can grow with the agency. Great opportunity to learn about compliance/ QAPI / COPs to move up to be part of the management team.

 

We are a small agency focused on patient centered care.

Responsibilities:

• Provide patient-centered care and case management services to assigned patients

• Conduct assessments and develop individualized care plans

• Coordinate and collaborate with interdisciplinary healthcare team members

• Monitor patient progress and adjust care plans as needed

• Educate patients and their families on disease management and treatment options

• Ensure compliance with all relevant regulations, including HIPAA

• Maintain accurate and up-to-date medical records

• Advocate for patients' rights and ensure they receive appropriate care

Requirements:

• Valid Registered Nurse (RN) license in the state of practice

• Minimum of 2 years of experience in a healthcare setting, preferably in case management in hospice

• Strong knowledge of managed care principles and practices

• Familiarity with medical terminology, procedures, and diagnoses

• Understanding of physiology and its application to patient care

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team

• Strong organizational and time management skills
